AstraZeneca and The Bloc for Agnostic
Care Management Toolkit Population Health Program

In collaboration with payers and the Commission for Case Manager Certification (CCMC), AstraZeneca developed brand-agnostic educational toolkits to improve outcomes for cancer, diabetes, cardiovascular, and respiratory disease patients. Nearly 500 toolkits were distributed over 12 months. Judges thought the toolkits were a great way to empower customers and patients. They also specifically liked the partnership with CCMC and the toolkits’ clarity.
